Abstract

Background Essential hypertension are major health problem in Thailand. People with uncontrolled hypertension often experience several complications that lead to disability and decrease their quality of life.

Objective This clinical trial study was to examine the effects of a Self-Management Program on self-management behaviors, disease control, and quality of life among Thais with hypertension.

Methods and Results A total of 184 people with hypertension participated the study but 172 completed the program. Subjects were people with hypertension who had health insurance and came for follow up visit at Kukot health center, Lumloogka and Zear brances. The program was developed based on Self-efficacy and Self-management theory. It consisted of task-specific self-efficacy for goal setting, action and monitoring progress, and making adjustment to attain the goals. The control group received the usual nursing care. Both groups received the same medical treatment. Data were analyzed using descriptive statistic, Chi-square, and repeated measure ANOVA. Finding indicated that at 6 and 12 months after enrollment, the experimental group demonstrated a statistically significant higher self-management behaviors score and quality of life than the control group. In addition the numbers of participants who reach the criteria of disease control had a statistically significant higher than the control group.

Conclusions It can be concluded that self-management program seems to be effective to promote disease controlled and quality of life in Thais with hypertension. Further studies should be replicated the effect of program on cardiovascular risk in a larger group.
